Trailblazers: The Paperwork

This page describes the forms you need to complete and submit. The forms are important for health and safety, for BSA insurance, and for other reasons. We ask everyone’s help in providing the paperwork as quickly as possible. We will have a table set up at the first two Troop meetings to collect forms and dues.

Special Paperwork Incentive -- Free Handbook! Any Trailblazer who turns in his Scout Application, dues check and Vehicle Information Form/Release before the first campout will receive a free Scout Handbook from the Troop.

Here’s what we need:

Scout Application (with Medical Info): To join Troop 5 officially, you and your son must complete a Boy Scout application.  Please be sure to fill in the medical information on the back of the application as well as the information on the front.

Dues.  Dues for new Scouts joining the troop are $40.  Checks are payable to “Troop 5.”

Vehicle Information Form/Release Form (single sheet):  We keep basic vehicle and insurance information on file for all leaders and parents because everyone is likely to drive to some scout-related activity. The other half of the form is a release form giving permission for your son to participate in scouting events and ride in someone else’s car. Troop 5 uses an annual release form. If you have specific concerns or questions about any event, please ask.

Ma-Ka-Ja-Wan Medical Form: We hope and expect that your son will attend Camp Ma-Ka-Ja-Wan with us this summer.  To facilitate that process, we’re handing out the Class 2 medical form now, which must be completed for him to attend.  Please note that a doctor needs to sign the form (or you may attach the signed State of Illinois medical form that was required at the start of 5th grade in lieu of having a doctor complete the Class 2 form).

Allergies, Disabilities and Other Conditions:  We strongly encourage you to advise the Scoutmaster if your son has an allergy, a disability, or any other physical, medical, emotional or other condition that is important to his health or care, or to working successfully with him in this program. Scouting is very inclusive and flexible, and we have youth with a variety of allergies, disabilities and other conditions.  However, it is important for the health and safety of all Scouts that the Scoutmaster be aware of any conditions that could affect your son’s experience adversely.
